Our next one will be held next Thursday, November 15, 2018 at 3 PM ET on Shared Decision-Making: Creating Partnerships in Care – The Joys and Challenges.

Many hospitals and primary and ambulatory care practices are looking for ways to engage patients and families meaningfully in their own care. Shared decision-making is a process of communication in which clinicians and patients work together to make optimal health care decisions that align with what matters most to patients. SDM is a means to the end of better health decisions. Many innovation efforts, including both the Transforming Clinical Practice and Patient Centered Medical Home Initiatives, are encouraging/requiring a shared decision-making effort. This informal conversation among PFCC.Connect members will discuss and explore ways different organizations are supporting shared decision making, leveraging partnerships with patient and family advisors and strengthening these partnerships. Come join the conversation to learn from others and to share your successes and challenges. Mary Minniti, Senior Policy and Program Specialist at IPFCC, will facilitate this discussion.
